Bill Contents 1. Cover Sheet. This sheet includes Client name/address, payroll periods billed, and the date the premium is due to ING Employee Benefits. Additionally, your Billing Administrator's name will appear on the cover sheet. Your Billing Administrator is available to answer any questions you might have during the remittance process. The cover sheet must be returned with all payments. 2. Remittance Worksheet. This sheet includes the payroll periods billed, the total amount billed, a list of exception codes, and an area to reconcile your payment based on exceptions. Once all exceptions are listed and total adjustments made, the total amount remitted should equal the total amount deducted from employee paychecks. 3. Change Report. This report is our primary tool of communication for employee's deduction amounts. The change report is printed on blue paper and includes any starts, stops and changes. The first bill will have all 'starts' on this section. Subsequent bills will have additional starts for late issues, stops for cancellations or terminations, and changes if a second policy is issued for an employee or an increase or decrease takes place. Please monitor this report upon receipt and adjust deduction amounts accordingly. 4. Full File Bill. The full file section will list current deductions for all participating employees.
