Using the Remittance worksheet
If a deduction is missed, and will not be made
up by payroll deduction:
Write the employee's name and ID on remittance
worksheet or draw a line through the employee's name on the bill and use
the "MD" (missed deduction) code. We will bill the policyowner
directly for the missed premium.
Differences in premium amounts:
If there is a discrepancy in what we billed and
what you are collecting, there is usually a reason. Please deduct amounts
currently on your bill and call your Billing Administrator.
If you are remitting premium for an employee not listed
on your bill, we do not encourage payroll centers to "make-up"
premium that is not billed unless the procedure is coordinated beforehand.
If you are deducting for employees not on the billing report, please contact
your Billing Administrator.
If an employee is leaving employment:
We can help your eligible employees retain their
insurance coverage by converting their policies to individual pay at the
same low, affordable rate. Please help us offer this benefit by providing
us with a current mailing address for any insured leaving your employment.
Please enter the employee's ID and name on the remittance worksheet and
add the "DB" exception code next to the name. When the payment
is remitted, we will immediately place this policy on direct billing to
the policyowner. Additional bills may be printed for your account before
we receive your changes. Therefore, please continue to make the change
until it is reflected on your invoices.
Cancellations and refunds
When employees request to cancel their policy,
please refer them to our customer service department at 1-800-537-5024.
A signature will be required. If additional premium is deducted after
a policy is cancelled, we will refund it directly to the employee's home
address within 10 days of receipt of the cancellation request.
If you wish to stop deductions upon the policyowner's/insured's
request, please notify us by phone, fax, or on the remittance worksheet
so that we may mail the appropriate paperwork to the policyowner/insured
and remove them from the bill.
